How to add Gratuity automatically to all the receipts before the customer is ready to pay and sign on the CC machine reader

Do you mean, you would like to add tips when you create an order before payment?
If so, you create an item and call it "TIPS" and add it to the order. You can leave the price of this item blank in the item's settings to be able to set up price (amount of the tips) upon sale.

Tips are calculated by percent for example total bill is $54 and system should have an option to add 15% tips to the total check and then give the total.

Could you not simply add as a "TAX" called "Gratuity" at say 10%?

Then it will be charged on every sale you make?

It would work as a workaround, or would the receipt say "TAX" rather than what you called it "Gratuity"

At the moment, you can add tips at the end of the purchase process if you using one of the integrated card payment systems: WorldPay, Yoco, Kicc, SmartPay, Sumup, PayPal Zettle. (It depends on the country where you run your business)
